What We Offer
The SC will connect the individual with all eligible programs and services
The SC will connect the individual to medical, social, educational and essential community resources
The SC will develop an Individualized Service plan and maintain in collaboration with the individual, their family and other team members.
The SC will provide consistent monitoring to ensure services are being delivered effectively and meeting the individual’s goals
The SC will meet with their Supervisor monthly to ensure cases are managed effectively and properly.
